Contact form and chat widget

When you use our contact form, you give us permission to use the information you provide to respond to your inquiry. The contact form may include your name, email address, ZIP code, selected topic and, where the form offers it, an optional phone number you may provide if you would like a call back. The consent statement shown beside the form describes exactly which contact channels you are agreeing to. Please do not include medical or diagnostic details in a form or chat message.

The LeadConnector chat widget is a separate contact channel. If you choose to provide a mobile number through that widget, its consent language explains whether you agree to informational, transactional or promotional calls and text messages, including messages sent using automated technology. Consent is not a condition of purchase, and the widget explains how to opt out of text messages.

We may use service providers to operate this website, transmit form submissions and chat messages, and help us respond to your inquiry. Those providers may process the information you submit on our behalf. We may also disclose information if you ask us to or when required by law.

Analytics
